Use this template at the end of each week to reflect on your progress with the 21-day decision capture habit. The four questions are designed to help you learn from your experience and adjust your approach.

What patterns emerged this week? What surprised me? What did I notice about my decisions or my day-to-day reality?

What actions did I take? What decisions did I make? What was the context? (Focus on what actually happened, not what you planned to do.)

Reminder: The goal is not perfection. The goal is to build the habit of capturing what actually happens so you can learn from it. If you miss a day, just pick up where you left off. The 21 days don't have to be consecutive.
